Form 1095-C Information

Who receives a Form 1095-C?

Applicable Large Employers (ALEs), or employers with 50 or more full-time equivalent employees, are required to send Form 1095-Cs to all full-time employees (those who work an average of 30 or more hours per week) as well as any employee who was enrolled in their health insurance plan. So if you were a full-time employee and/or were enrolled in health insurance through your employer at any time during this tax year, you should receive a Form 1095-C.

Why is the dollar amount in Part 2, Line 15 not the same amount I pay for medical insurance?

This amount is the monthly employee contribution for a single coverage pan in the lowest-cost plan offered to you. The IRS uses this amount to determine if your employer offers an “affordable” plan that meets the requirements of the ACA law. You may have actually enrolled in a different plan and/or you may cover family members under a different plan that may be more expensive.
